Anniversary is an occasion for new breakthroughs

For the Semashko Republican Clinical Hospital, the 95th anniversary is, first of all, an assessment of the work done and the identification of new goals for moving forward.
About what these results and goals are - our conversation with the chief physician of the Republican Hospital Evgenia Ludupova - Evgenia Yuryevna, 95 is a solid figure.
What does it mean for you personally and for your team? — An anniversary for us is not fanfare and a magnificent feast, but, first of all, an assessment of ourselves at a certain stage - what we have achieved, what heights we have reached. This is planning for future goals. This is a new stage on the way to the 100th anniversary of the hospital. This is also a reason for new initiatives, directly related to our main work.

As part of the anniversary year, we launched the “95 good deeds” campaign. As part of this action, for example, our vascular center, together with other doctors, prepared the “Republican Hospital for Primary Care” program. We all understand perfectly well: to become a professional doctor, you need to constantly learn and grow professionally within yourself. Therefore, in the context of this program, every Saturday our practicing doctors - endocrinologists, neurologists, cardiologists - will conduct various interesting lectures in the format of a training school, inviting primary care doctors from the region to them. So that our doctors have the opportunity to update their knowledge, we have raised the old school, which was always the recognized leading professors of the St. Petersburg and Moscow schools - they constantly conducted “analysis of patient histories.”

With those who cannot come from the region, we will talk via video conferencing about interesting and difficult cases from practice, about new approaches to treatment. The program plan has already been outlined, and by the end of the year the doors of our hospital will be open to primary care doctors throughout the republic. And in the future I would like to make this school a permanent platform for learning in the “eyetoeye” format - eye to eye. Currently, we are the training base for students of both the medical college and the medical institute. But now we want to move on, in new formats.

— It is known that you attach paramount importance to the training of your staff...

- It can not be in any other way. “Lifelong continuous education” is a kind of trend today in medicine. That is why we constantly hold conferences in our hospital, to which we invite leading specialists from all over the country. We recently held three conferences. At one of them, almost all Russian gastroenterologists gathered, who shared their experience and analyzed all the progressive achievements in this area. The second forum, to which leading specialists from our country were also invited, was dedicated to the 50th anniversary of the cardiological service of the Republican Hospital. And here we have something to say, given that our cardiac surgeons, who can rightfully be called heroes, perform up to 250 successful operations a year. The third conference was devoted to an equally important topic - research in the field of microbiology.

Taking into account the fact that a regional Competence Center for the quality and safety of medical activities has been created on the basis of our hospital, where we also collect the best practices in Russia and around the world, in June we successfully held the annual conference on quality management for the fifth time. At this event we organized 7 sections, interesting speakers came from Moscow, Kazan, Krasnoyarsk, Tyumen - leading industry leaders who have developed their good practices.

I would like to emphasize that we never hold events of this kind out of any ambitious motives; they are very important in a practical context - after all, nothing enriches a doctor’s knowledge base more than exchanging experiences with his colleagues. Without summing up, without exchanging opinions, I believe it is impossible to move and develop further in our constantly changing healthcare system.

In addition, fully understanding the problems of young doctors who today do not have the opportunity to study in an internship and immediately after graduating from universities come to the hospital, we provide them with support, sharing our accumulated experience as much as possible.

— During the Soviet period, the development of the entire country was planned and included in the so-called “five-year plans.” Do you also calculate your progress from anniversary to anniversary?

- That's probably true. The anniversary is another reason for us to think about what we have already achieved and what we can still achieve. Five years ago, when the Republican Hospital turned 90 years old, we voiced our dream - to completely switch to digital mode by the 100th anniversary. To make it convenient for everyone, both doctors and our patients. So that we can transmit information about their health to our patients through a secure channel, so that our queues are reduced to a minimum. Because when it is possible to transmit information via electronic networks at high speed, the treatment process is accelerated many times over.

And now, assessing our work, we can say with confidence that our team has done a lot over these five years. Firstly, we have created a database of our patients, and now doctors only need one “click” to open the medical history of those who come to us for the second time. Secondly, we have created a central archive of digital images - the doctor can now examine any x-ray image in digital mode, rather than running through the corridors looking for it. Thirdly, every doctor can now instantly see test results online on his monitor - we have created a laboratory information system. Our laboratory performs 560 types of analyzes, and five years ago there were 380 types. The dynamics of growth are obvious, and so is the dynamics of digitalization. And all this means that doctors have a significantly reduced time for completing documentation and studying the situation, and accordingly, there is an opportunity for better examination and care of patients.

And, of course, our further task is to integrate with the entire system of the republic and Russia, and we are moving towards this. The task, of course, is complex, but this is a priority project of the government of the Russian Federation - the creation of a single digital circuit. In this regard, the goals of our hospital coincide with the goals that the state has set for us. And I think that at the Republican Hospital we have one of the best practices regarding the implementation of digital technologies.

— It happens like this: when a team is given some kind of super task, at first they don’t believe that it can be brought to life...

“Now we are faced with just such a maximum task. It was put before our team by the head of the republic, Aleksey Sambuevich Tsydenov, this interesting idea is supported by an initiative group of our surgeons, including our colleague, deputy of the People’s Khural Galan Dambievich Gunzynov. This is a completely new direction for our region - the development of transplantology. We have been nurturing this idea for 5 years now and gradually, by studying the experience of other territories, we are preparing to bring it to life. Transplantation is a very pressing problem for our people, since the number of patients on hemodialysis is growing every year. Up to 80 people in Buryatia annually, unfortunately, are forced, due to the development of the disease, to switch to an “artificial kidney”, and in general, today there are about 500 patients on hemodialysis. In addition, quite a lot of patients suffer from liver diseases. In a word, for both of them the only way out is organ transplantation.

Of course, bringing this direction to life will require significant financial investments. Firstly, for the training of doctors, and secondly, for the purchase of necessary medical equipment. And if our operating rooms meet all the requirements of this type of operation, then at least 50 million rubles will be required to open and equip a morphological diagnostic laboratory. And most importantly, since in general the topic of transplantation itself is delicate and has very fine moral facets, understanding of society, support of religious faiths, and legal support are necessary. Considering the rigidity of the approach to this topic and the global nature of the task set before us, it will be necessary to weigh everything down to the smallest detail and regulate each step. And there is another fine line here. Of course, the surgeon’s hands must be golden, but a favorable outcome largely depends on the selection of the donor-recipient. In a word, I think we will still need at least about two years to adopt all the best experience available in this area. And there are no small details expected in this innovation.

— Does the team of the Republican Hospital have a most cherished dream?

— The cherished dream of our entire team is a new hospital for the 100th anniversary of the medical institution in Buryatia, which has the richest traditions and the richest experience in the republic. Today the perinatal center celebrates its half-century anniversary, and the children's clinical hospital is successfully functioning. But these medical institutions set off on their own, emerging from the walls of our Republican Hospital. And we are happy for our colleagues who work today in such comfortable conditions. Of course, our hospital also does not go unnoticed. With a subsidy as part of the Far East development program, already this year we will be able, for example, to bring the facades of our branches into proper shape. But as for global modernization, there is little we can do here due to the fact that most of the buildings were built in the first half of the last century, accordingly, all the floors in them are wooden, and we can’t even talk about installing ventilation, elevators or bathrooms in each room has to. How uncomfortable it is when the air composition does not meet the requirements of certain standards, when nurses and patients themselves are forced to carry bedpans across the entire corridor, and what efforts are required to transfer seriously overweight patients from floor to floor, it is clear without words. Of course, we are making every effort to somehow improve every corner of our hospital in order to give it a decent appearance. And when some supervisory authorities come to us with inspections and say: “Your place is poor, but clean,” we are simply forced to take this as a compliment to our futile efforts.

In general, it is necessary to launch the second stage of construction of our Republican Clinical Hospital. As part of the second phase, two surgical blocks with 300 beds, a level 1 trauma center with 180 beds, a block with 24 operating rooms will be built, the clinic will be reconstructed and expanded. In fact, this will be a new hospital that will provide high-tech care to all residents of not only Buryatia, but also the entire Far East.

The commissioning of new buildings will open up new areas of medical care in the field of emergency cardiovascular surgery, arrhythmology and organ and tissue transplantation. Doctors will be able to perform even more operations per year and provide a larger volume of therapeutic treatment, which means saving and restoring health to even more residents of Buryatia and the Far East.

New buildings with state-of-the-art equipment will attract many residents of European and Asian countries for treatment, because we have a treasure - Lake Baikal. Coupled with a high level of medical care, this will give a powerful healing effect.

The construction of the second stage will be an invaluable gift for the 100th anniversary of the Republican Hospital in 2024. We will not only be happy, but also ready for new breakthroughs and achievements.
